Who can benefit from a skilled liposuction procedure?  A large range of patient needs can be met with liposuction.  Among others, some of the following needs can often be addressed.  1) Many patients benefit from liposuction underneath the chin.  Sometimes this procedure can produce results that patients thought could only be achieved with a facelift.  2) Breast reduction can be achieved for women with large or uneven breasts.  Men with gynecomastia (“man boobs”) can also undergo liposuction instead of or in addition to excision procedures.  3) Abdominal liposuction is a commonly-requested procedure, used by both men and women to remove excess tummy fat.  It is much easier and safer than a full tummy tuck procedure, and should definitely be considered when the patient wants to reshape the tummy area.  4)  The hips and buttocks can be treated with liposuction, to remove stubborn fat that is not coming off with diet and exercise.  5) Some obese patients who have already started their weight-loss program can get a morale boost and health benefits from large-volume liposuction.  5)  People who need small-area reshaping or touchup procedures can benefit from “microliposuction”.

Over versus under-the-muscle breast implant placement

Reading the Internet, the average patient might assume that the only proper placement of a breast implant is under the muscle. However, like much in life, this has its pros and cons.

We like breast implants that are placed over the muscle. We do have a special technique where the implant is put under the skin of the muscle, which provides some of the advantages of under-the-muscle placement. In theory, this is designed to prevent a sharp demarcation or “half an orange” look in the upper pole of the breast. This subfascial (sub skin) of the muscle placement does provide a result that prevents this to some degree.

If you have a lot of breast tissue, and relatively perky breasts without a lot of sag (which may require a lift with scars on the breast) the tradeoff seems relatively clear to me, that over-the-muscle rather than under-the-muscle placement provides a very nice effect. Note that for under-the-muscle placement, he muscle needs to be cut in its lower third in order to prevent a lot of strange outcomes, so under-the-muscle placement really is under the muscle only in the top part of the breast implant. This cut of the lower third of the muscle can produce bleeding and other problems in some cases.

The advantages of over-the-muscle placement are as follows:

Number 1, cleavage can’t be as close under the muscle because the muscle attaches to the bone in the middle of the chest or sternum and the breasts can only get so close.

Number 2, any active woman who has under-the-muscle placement has a constant push on the implant, down and out, and eventually, when the woman lies on her back, her breast implants are displaced toward the side and are not as attractive because the pocket stretches out on the side and towards the bottom of the implant. In some cases, the implant may actually descend.

Number 3, after pregnancy, the implant/breast combination may have a look much like a waterfall, where the breast will fall off an implant which is under the muscle. This implant is fixed in such a way that the breast, which enlarges during pregnancy and then softens, will generally fall off of the under-the-muscle placement. Some medical journal articles say this happens in as much as 75 per cent of cases. SEE ACCOMPANYING PHOTO.

Number 4, there can be a complication called a “seroma” or fluid collection around the implant. This complication sometimes requires removal of the scar around the breast implant in some cases. This is a big hassle to do under the muscle and sometimes these seromas become chronic and are a significant problem in the long term.

Number 5, the relatively rare (though possibly as much as a half per cent on some reference websites) complication of a collapsed lung is very, very rare with above the muscle implants. This complication isn’t usually very serious but can result in a hospital stay of a couple of days with a chest tube, an uncomfortable and expensive hassle.

The under-the-muscle placement in a young person with relatively full breasts can indeed produce a very nice result in the intermediate (five to ten year) term, especially when the breasts are relatively tight and there is a significant amount of breast tissue. However, over the long term, these other problems can develop, and it’s important that we make you aware of them.

The advantage of over-the-muscle implant is possibly less droop of the implant over the short to intermediate term as the heavy implant responds to gravity and forces the skin and breast tissue to stretch. But if you wear proper bras (we recommend the Bali Minimizer style) most of the time, you can likely retain the breasts you see right after surgery almost indefinitely. Wear these bras even if you get over the muscle implants, as directed by your surgeon.

All that being said, after your own research, you are free to make your own choice, but take our counseling into account. We have photographs of these complications for you to view at your consultation.

In experienced hands, this technique works wonders for many patients. It seems to have a lower rate of complications, such as capsular contraction, which is firming of the breasts. This complication accounts for the highest of all of the problem rates in breast augmentation surgery.

Dr. Peter Cheski pioneered and invented this technique, and Dr. Yoho learned his technique right from Dr. Cheski, working closely alongside him in his office over the years. Dr. Cheski was at the point where he could perform one of these surgeries in 10 to 15 minutes with virtually no help outside of a scrub tech and a sedation person who watched the monitor. He performed many tens of thousands of these surgeries over the years, and Dr. Cheski is regarded as one of the pioneers in breast augmentation.

Dr. Yoho would like everyone in Pasadena, Los Angeles who has been curious about Botox, to have a chance at trying it out. The best candidate is usually under 40 years old, with tight, elastic skin plus fat deposits. These people typically get the most successful results. A second group has skin that might do better with a lower facelift, but for reasons of cost, discomfort, recovery or risk, they want a simpler procedure. This sort of person might decide to have other minor treatments such as medical spa or a chin or cheek implant in combination with the liposuction. Some of our Pasadena, Los Angeles cosmetic surgery patients need other eyelid procedures to smooth out the wrinkles of the lower lid. There are two options: laser resurfacing (laser peeling), or taking a little pinch of skin from the lower lid near the lashes. Lower eyelid surgery without skin resurfacing has a much easier recovery, but the results aren’t as consistent. Some patients choose to have their eyelid surgery and come back later to have resurfacing when they have more time. Eyelid surgery patients should use ice compresses for the first 24-hour period and most require only two to three days off work if they don’t have resurfacing and about a week if they do.

Other Upper Face Contouring Options – Los Angeles Eyelid Surgery

Some people have a very narrow space between their eyebrows and eyelashes and they can benefit from having a little bit — sometimes only 1/8th of an inch — of elevation of their brows. Brow lifts go along with eyelid lifts because a careful brow lift can make an eyelid lift much more spectacular. The modern brow lift is done through a few incisions in the scalp. Brow lift patients do have some pain after the procedure and may need codeine-type painkillers for about 2 days after the surgery.

I was at a conference where liposuction was being demonstrated. Patrick Lillis M.D., the dermatologic surgeon who invented this special technique, had a patient who canceled his surgery. Dr. Lillis offered to let me be his patient.  I agreed, and the experience was unusual but not all that unpleasant.  Dr. Lillis performed the procedure while I was entirely awake. He inserted the local anesthetic through my skin, which was almost painless. I lay there and looked up at the crowd of surgeons watching me.  This video was taken of the procedure and at one point in the video, it looked as if I was trying to suck my thumb….

In any case, the surgery was soon over.  I had a few uncomfortable moments but I did very well. About a half a quart of fat was suctioned from my midsection.  I was placed in a waist wrap and then stood up and joined the rest of the participants watching other surgeries. I finished the day on my feet without any problems.  My recovery was easy. Ever since then, I have had a “tummy” six-pack, and I show people my abdomen if they ask me about my own liposuction.

Note: our patients almost always receive sedation so they do not remember the procedure, but if requested, we can perform “pure local anesthetic liposuction” just like I had.

I brought one of the first fractionated CO2 lasers to my office in Pasadena. I decided the SmartXide DOT® laser was the best. Soon after it’s purchase, I decided to see what its effects would be on my own complexion.  I had some dark spots, large pores and a slightly gray complexion tone. This is of a new generation of CO2 lasers. Tiny laser beams treat the involved surface. Bridges of skin in between the treated areas remain, thus allowing a very quick recovery.  My experience follows.  Just like my patients, I scrubbed my face with alcohol, and applied a “triple-numbing” cream and waited about an hour. Then my physician assistant used the laser on my face for about 20 minutes.  When he was done–and it did hurt a bit–I experienced a little bit of burning for a few hours, which I ignored. My recovery was complete by four days.  Tiny flakes of skin peeled off my face and I would wash a couple of times a day to hydrate and promote the healing.  The result: smoother skin with a clear complexion and reduction in pore size. I repeated the laser procedure at two weeks intervals for a total of three treatments. You can see my result in the accompanying photographs. My laser experience was no miracle,there is a  facelift effect but I needed no time off work and had almost no discomfort or unsightly appearance. Although the procedure is reasonably comfortable and does not usually require relaxation medicines, we provide this at no extra charge if requested. A driver is necessary in this case.
